1 dribble hand off with 2 and then continues to a baseline screen. 1 bumps back off of screen from 5.

After receiving pass from 2, 1 comes off ball screen and pop from 4 (4 is shooter). 5 is there for a drop off pass and 2 and 3 spot up.
